| Most dentists are actively trying to increase the size of their practice - and we've decided to actively help these dentists with their goals. | FREE Patient Satisfaction Surveys |
| You can receive emailed Patient Satisfaction Surveys from your patients after their visit to your office. |
| We'll assist you in setting up the Patient Surveys with up to 10 questions about your office. |
|
"The information that you get from your patients' surveys is important information for your staff meetings if you want to improve your practice without wasting time, money and credibility guessing at what you think needs to be 'fixed'." |
|
| There's even a "stats sheet" that will give you a cumulative averages for each question's score. So you can get an average score for how your patients are ranking each of your concerns AND that can help measure your efforts to improve. |
| Call us and we'll set it up with you or a staff member so that you can see how it works before you decided. It takes about 10 minutes to set it all up and then we can pretend that we are a patient taking your survey. |
| Please note: you must be offering the dental grants (#1) in order to receive the emailed Patient Satisfaction Surveys - the dental grants are the motivation for the patients to answer your 10-question surveys. |

We offer free "Before-and-After" photos that you can frame and hang in your office. The framed photos will help initiate discussion about cosmetic work that the patient might not have thought was feasible (and the photos help the patients understand how the photo process works). |

| More and more patients are looking for dentists online - make sure your name is there when their looking. You can be listed on multiple internet dental directories with no effort on your part - we do the work. We offer basic listing at no charge - regardless of which lab you use. |
| We list your practice in multiple internet directories so new patients find your office. The local listing/s can include a full custom 'practice profile' with info and photos for your practice and can be linked to your own web site. |

FEES FOR PRACTICE BUILDING PROGRAMS: |

When you send an average of $1,500 per month in lab work, you may use the AmericaSmiles Program (with Full-Listing / Practice Profile / Dental Grants / Patient Surveys) and / or the Custom Web site (with hosting & support) and / or the Smile Design Photos and / or the Restoration Warranty Program at no cost to you. |

If your office sends some lab work, but fails to maintain the $1,500 average, we reserve the right to invoice you a single charge totaling $35 per month for the Web Site and/or any portion of the AmericaSmiles program and/or the Restoration Warranty Program. The Smile Design Photos would be invoiced individually. |

For dental offices that want to continue using their current lab/s - and send no lab work to Artistic Dental Design; but want the AmericaSmiles program (as described above) and/ or the Custom Web Site with ongoing hosting and support, and / or the Restoration Warranty Program, the total fee for those services will be as follows - your option: |

Annual: $480 billed once per year (= $40 per month) OR |
|
|
| (Who else can you hire to work for your practice recruiting new patients all year for less than $500?) |

$270 billed once per 6 months (=$45 per month) OR |
