We occasionally run a "We Care What You Think" drawing.

When we're running a drawing, the Case Evaluation Forms that are sent back to us may win great prizes like . . .

The bottom portion of each invoice (sent with each case), is a Case Evaluation Form. After you deliver the case to the pt, rate about 10 aspects of the case by putting an "x" in one of three boxes and return the form to us with another case or by mail.
Give us your honest opinions about our work - THAT'S IT!
When we're running a drawing: each Case Evaluation Form that you return will be used as a separate entry.
Whether we're running a drawing or not, you'll win over and over - because the more we receive your comments, the easier it will be for us to consistently meet your expectations for lab work.
The prizes you may win will just be "icing on the cake"!
Our single goal is for you and your patients to be completely satisfied - EVERY TIME - EVERY CASE.
